Price per square foot is an output, not an input. Add up concrete (with overage), gravel base, reinforcement, forms, labor to form, place and finish, plus demo and haul-off if needed. Divide by (1 − margin), then divide by the square footage.
Why small slabs cost more per square foot
Mobilization, short-load fees and minimum crew time don't shrink with the slab. That's why your per-square-foot price should rise on small jobs.
FAQ

Does demo change the price a lot?
Yes. Removing and hauling old concrete adds labor and disposal costs.
